Writing a Statement for Small Claims Court?

I am confused with the way I should write the statement for my small claims session. I got my legs waxed at a nail salon and my legs got all burnt and bruised. I have to write a statement of why I would like to sue them I just don’t know a good way to write it. Any suggestions on how I might word it?

On the Small Claims Lawsuit form where you are asked to recite the events and actions leading to your injury you might consider using the following language:

On the___ day of ____, 2012 while a customer at the _____________ Nail Salon I suffered burns and bruising about both of my legs.

At or about the time of the burning and bruising I was a customer who was invited into the salon by the salon’s advertisements (or word of mouth), which ever applies.

At or about the time of the incident I had no other burns or bruising on my legs. I was not pre-warned about the possibility of burning or bruising from the wax treatment. As a result of the incident I suffered pain and discomfort.

Because of this incident I am asking the court to award me the amount of $__________ against the owners and management of the salon.
